Yeah for sure but in this case we are talking about inbreeding as a practice of keeping pure bloodlines and its disastrous consequences which itself IS a choice.
Now if it were used to mock people of small isolated populations for whom it is a genuine concern that’s a whole different problem and topic but in the case of the OP I don’t think it is at all ableist.
i think you need a second thing to make inbred ableist and that part is generally going to be ableist on its own too.
“inbred hick” is probably punching down and not a generational purity thing, “inbred nobles” is punching up, and i’d argue it’s not really about one guy but the whole family system and how they all behave because of their class position.

The problem with inbreeding is that it leads to disabilities. There’s not really any moral reasons not to have sex your cousins that you wouldn’t also have with a childhood friend.
So for it to work as an insult would mean it’s implying that they are disabled.
first cousin pairing doesn’t cause any health problems or deformities unless you do it a lot for several generations

yes it would have an ableist term in it where it did not before and would be referring to whatever that was instead of the tendency of old noble families to inbreed for generations out of some proto eugenic purity ideals.
I’m gonna approach this from a different angle. I grew up in a rural part of north America where the term “inbred” was synonymous with mentally handicapped. I actually know a lot of inbred people. Hence why I was the first here to point out that it’s a slur.
